What do I need to bring with me to my appointment?

In addition to bringing your insurance cards, photo ID and referral information, please bring a list of current medications with you to your appointment, including the dosage. If you are a new patient, you may download copies of the new patient paperwork from this website and bring them with you as well. This will help minimize your wait time on the day of your visit.
